Submit a scanned form

After you complete your blue card application form, you can scan it and send the scanned version to us using this online form.

Only 1 form will be accepted per submission. If you have more than 1 form, you will need to fill out the form on this page for each of the forms you need to submit.

If payment is required, you need to pay online before you submit your form. Read about payment information if you’re not sure if you need to pay.

Please note that processing and approval times may take longer for paper applications submitted in this way.

If you would prefer to submit your application using the online portal instead, you can register for an online account (Opens in new window) and submit your form to us that way. Registering for an online account means you don’t have to provide a certified copy of your Customer Reference Number or identification.

  1. Does your form require payment of a fee?
  2. Upload a form that does not require payment of a fee (If you chose 'No' above)
  3. Upload a form that requires payment of a fee and you have already paid online (If you chose 'Yes' above)
