    • the event is attended by more than 100 people, and
    • the work is for 10 days or less on no more than two occasions per year; and
    • the person is unlikely to be physically present with a child without another adult being present.
  • a child under 18 years of age volunteering (except “trainee students” undertaking a course of study with an “education provider”)

  • providing services as a crossing supervisor within the meaning of section 122 of the Transport Operations (Road Use Management) Act 1995,

for at least:

  • eight consecutive days, or

  • once a fortnight, each fortnight, over eight weeks, or
  • once a month, each month over six months.

How to apply

If you provide services as a school crossing supervisor with the Department of Transport and you need a blue card, you need to complete the blue card application forms which have been pre-populated specifically for the Department of Transport School Crossing Supervisor program. These are the only forms to be used when applying to obtain and renew their blue cards.
To access the relevant application form please use the links below.

How to renew your blue card

The Commission will send you a renewal invitation 16 weeks before your blue card is due to expire. To ensure that you get the reminder, it is important that you inform the Commission of any change in your address or other personal details.

You may only renew your blue card if you are still in child-related employment. If you no longer require a blue card, you are unable to renew at this time. You are welcome to re-apply at a later date if you require a blue card in the future.

A renewal blue card application form has been pre-populated specifically for Queensland Transport. You will need to obtain this form by contacting your Road Safety Supervisor.
