Statutory declarations

Where a person needs to make a declaration, for example where they are carrying on a business on a non-profit basis and is requesting that the application fee be waived, please submit the following Statutory Declaration providing details about the reason for the declaration.

Director of a governing body of a non-state school

A person who is a Director of a governing body of a non-state school is deemed to be carrying on a regulated business and must submit a B form accompanied by the following Statutory Declaration so the Commission can waive the $73.45 fee.

Change of name

When submitting a Change of name form with the Commission, you should also attach a certified copy of the document evidencing your name change (eg Marriage Certificate or Change of Name Certificate). If you do not have a document which evidences your name change you should complete the following statutory declaration and attach it to your change of name form. You will need your Statutory Declaration witnessed by a Justice of the Peace (JP).
